I think we saw it as just a stand-alone post.
MS Outlook is
HOOOOOOOOOORRRRRRRRRRRRRRIIIIIIIIBBBBBBBBBBBLLLLLLLLLLLYYYYYYYYYYY
brain-damaged about all sorts of things. That, along with it's alter-ego
MSIE.
In this case, it sounds like they ignored the RFC's and such and threaded
on just the subject string instead of following the 'References:' header
lines.
Sounds very similar to the behavior of MSIE when it gets a HTTP transfer
with a MIME-Type, but with a "file extension" that the OS has
registered...

Agent is broken this way too. It also loses threading when someone
changes the subject line, and it forces one to read crossposts in
every group. If it weren't for the ability to look up messages by
message-id, I'd think they'd never heard of them.

Ron, will you please re-read the docs on these two specific option
tabs:
Group->Default Properties->Crossposts tab
and
Options->General Preferences->Message List tab
I think it will work for you. It does for me.

I may be using an old version of Agent, from back when they said they
had those features but they didn't actually work. I'll check my
settings again when I get home.

Ron, I used to have my group Properties set to skip the header if it
had already been retrieved in another group. I actually changed it back
to be able to see problematic crossposting from visitors. Right-click on
any group folder to set Properties.

Better yet, set them globally (there are a lot of groups to think
about). The "Start new thread when subject changes" checkbox might be
what's causing you trouble, as well as the "Enable threading by
subject" which, as I understand the help file, overrides the
Reference: header field threading.

Be happy! The Outlook Express 5.0 for Mac OS version has the threading
feature removed (!!!) because "newsgroups are dead"! So you better hold on
to your current Outlook Express version as long as the Windows version still
has this feature!
