Howdy,

Not 100% happy with this but it's almost there .....

Besides the light bulb in the lamp, theres a light (shadowless) with short 
fade distance up and behind the camera postion.

I used focal blur and a touch of emmission/scattering media inside a cone 
for the affect from the lamp.

What am I missing?  A few more items on the table?
