Let T(v) be the result of applying the transformation to v.
Then the matrix of T is

   T(x)-T(0)
   T(y)-T(0)
   T(z)-T(0)
   T(0)

where each line should be read as a horizontal 3D-vector.
At least that is how I read 6.3.1.4. I have never used any of this.
