Should be possible with world machine to import some rough sketch
as input height field, then combine it with random terrain and erode
it later. But the basic (free) edition is limited to 512x512.

Yes indeed. WorldMachine would do something similar, except for the size. I 
use here 2048x2048. The tga file is 48 MB large.

It has something appealing, I agree. However, including figures would be 
difficult as they would become stretched too...
