Simple answer, the actual pens I used for reference did not have them.

    I was going to post something about the holes being conical, not
cylindrical, but, all the biros I have here are without hole...

Are you using post processing focal blur or the built in (slow) focal blur?

Im using the good old slow focal blur.
This image took 21 hours to render, although I was using a rather
large (8x8) area light.

Since there also isn't a hole in the base plug (such as what some recent
pens use) I'd guess that the reference pens don't work too well. As the
ink gets used up, the air pressure inside the pen decreases and resists
the flow of more ink.

Actually after closer examination the pens have 2 little air holes
in the nib (just where it joins to the main case).
I will have to add this.

I did some test renders with radiosity but got some strange effects around
the steps in the nib of the pen. It appeared as if there was a light source
inside and it was escaping at the joins. Also the render times for this
image
are quite high already if I did fix the radiosity problems I think it would
take about a month to render :)
