The lighting in this is completely wrong... (I also did a metallic 
version; the lighting was completely wrong there too.) Also, I cannot 
make the glass look refractive.

Oh well! It's not a terribly exciting scene anyway. :-/

the highlights on the plane from the refraction are very cool, you should
use them to improve the lighting. Also, contrast is too low i think because
everything seems to get lost in the black background. maybe add some small
sized phong to the texture to simulate a polished glass surface.
Maybe you could extend the plane so the background is not totally dark. I
like this image!

For refraction to show, it need something to get refracted. Your background is to
plain and tend to 
hide any refraction effect. Try without any reflection as the reflection may hide what
is refracted. 
Once the refraction is OK, you can put back the reflections, I suggere using variable
reflection 
with fresnel, adding conserve_energy in the finish. You may want to increase the ior
to get a more 
pleasent/artistic effect. If you use specular highlight, try to lower the roughness,
probably under 
0.001 (small value = small highlights). If you use phong, increase phong_size, maybe
over 200 (large 
value = small highlights).

Still... I think this is an enjoyable image. The objects are well done.
There should be something to refract, though...

Well, now there's something to refract... and yet still it doesn't look 
all that great. :-(

I think I need to do something drastic to the lighting; it's just too 
dam dark!

You can try replacing the pigment with rgbt 1, then using fade_color and fade_distance
in the 
interior. Place the colour from the pigment as the fade_color, without the filter or
transmit part. 
Increasing the intencity of the light may effectively help. Maybe a little less
reflection.
