The camera is different from a real world camera. For most
purposes the aperture is simply a point (point camera). Very
simple optics, in focus everywhere, and the raytracer does not
care that almost no light would pass through in real life ;)

The latter. See 3.4.2.3 in the documentation (of POV-Ray 3.7).

In 3.7 there is also a bokeh keyword to set the shape.
