Don't know about the other issues but I am pretty sure that if you surround 
the scene (camera in particular) with an object you can use that to set your 
"background" ior. Something like:

plane {y, 100000
pigment {rgbt <1, 1, 1, 1>}
interior {ior 1.3}
}
