Outch indeed ;-)
I think your image needs an hdri environment to reflect into the tray. The
metal is looking very *flat*; maybe it needs (much) less reflection and more
specular. The wood needs a bit more work too.

I'd turn off the ambient light altogether and use some radiosity for starters,
and then use the hdri for even better results.

Nice start, really painful idea, and ditto to all the other suggestions.
Composition-wise, maybe put the tooth on some saturated, bloody gauze to
emphasize that aspect of the pain...
